Description
Reporting to the Home Manager, the Dementia Care Lead is responsible for ensuring a high level of dementia care is provided to our residents and meets the code of conduct and company procedures.
The Woodlands Unit Manager in our Dementia village will lead the home teams in providing strong dementia knowledge and leadership to ensure the highest standards are achieved in all areas of the home including relationship centered care.
Key Responsibilities
Leadership and Management
- To be responsible for the provision of innovative and expert dementia care within the unit, to ensure that people with dementia are helped to maintain/regain their independence, improving their sense of well-being.
- To be available to relatives to offer support and guidance on dementia care.
- To lead and empower the team to provide relationship centred care to all residents living at the home ensuring the teams provide meaningful occupation at every opportunity.
- Manage the recruitment and training of the care team; ensuring training targets are met and the team is competent in their roles.
- Responsibility for the supervision, appraisal and performance management of the care team, supporting them to achieve their potential.
- To ensure appropriate training is available to accelerate their learning for all team members Home in the provision of Dementia Care and monitored to ensure it is effective.
- To administrate and organise own work to ensure that it is accurate and meets quality targets, reasonable deadlines and reporting requirements.
- To assist with the promotion of SilverCrest Care Homes
Dementia Care
- To work and maintain care practices, policy documents and procedures alongside our quality audit and assurance programmes.
- To advise on developments of research based practice in relation to Dementia Care.
- To liaise with commissioners and care professionals of partner organisations to ensure care requirements of older people with dementia and best practice are understood and met.
- To co-operate with internal and external audits as well as conducting home specific audits based on care plans, medications etc.
- To play an active part in research initiatives internal and external, which may advance knowledge expertise and care delivery.
- To be actively involved in raising standards in care projects for residents living with dementia by undertaking assessment formulating action plans and being active in moving care projects forward to reach SilverCrest’s high standards of care to ensure that SilverCrest are a leaders in this field.
- To ensure the security of all information regarding all staff and residents
- To develop networks with partner organisations who also work in the field of dementia, mental health and disabilities and to represent the needs of people living with dementia at local forums/events.
- To manage referrals, assisting clients in assessing their information and support needs.
- To advise the team of the requirements and consistent implementation of the Care Standards Act and other related legislation/policies in relation to care delivery.
- To develop a proactive approach in reaching people with dementia.
- To seek and record client feedback to understand whether needs are being achieved, and to plan any necessary adjustments.
- To work in a manner that facilitates inclusion.
- To actively engage with residents in conversation and meaningful occupation related to their lifestyle choices at a level and pace that values the individual and respects their dignity and communication differences.
- To respond in a timely way to residents who are distressed (calling out, calling for help, knocking or making noises that suggest the need for support) or seek assistance if you feel unable to respond appropriately.

Experience
- Minimum of two years shift leadership and management in a care home
- Proven track record in a care home environment delivering quality care
- Proven track record leading, empowering, supporting and motivating a clinical team
- Experience delivering high quality relationship centered care (Desirable)
- Dementia care experience (desirable)
Skills/Knowledge/Qualifications
Qualifications and Education
- RN/RGN Qualified or RMN with strong clinical experience
- Dementia Qualification for Dementia Care Homes
